EECP (Enhanced External Counterpulsation) is an FDA-approved, non-surgical treatment method that aims to support blood flow to the heart using cuffs wrapped around the legs.
The standard protocol consists of a total of 35 sessions, 5 days a week, with sessions of about 1 hour each day, lasting approximately 7 weeks.
EECP does not require anesthesia, needles, or a surgical incision. During the session, the pressure from the cuffs may cause a temporary feeling of tightness in the legs; this is generally not painful, and the pressure level can be adjusted for comfort.
It is considered for conditions such as angina, coronary artery disease, and heart failure, particularly for patients who do not respond adequately to medication or who are not suitable candidates for surgery.
EECP is not recommended, or requires careful evaluation, in conditions such as pregnancy, aortic regurgitation, atrial fibrillation, a history of blood clots, uncontrolled high blood pressure, and severe peripheral vascular disease.
EECP has been the subject of scientific research for more than 30 years, including the MUST-EECP randomized controlled trial, the IEPR patient registry, and various systematic reviews. It is listed in ACC/AHA guidelines as a Class IIb recommendation (Level of Evidence B) for patients with refractory angina.
Follow-up data from patient registries show that the benefit observed after treatment persists over time in some patients; however, this can vary from person to person and may not result the same way in every patient.
