What Is Refractory Angina?

Refractory angina is defined as recurring chest pain or pressure that persists despite medication and standard interventional methods (such as angioplasty or bypass). In these patients, the structure of the coronary vessels is often not suitable for an additional stent or bypass procedure.

What Options Are Available for Refractory Angina Patients?

Treatment for refractory angina may involve medication optimization, lifestyle adjustments, certain specialized interventional methods, and non-invasive options such as EECP. EECP aims to support blood flow to the heart through synchronized pressure applied to the legs, without requiring an additional surgical procedure.

The Level of Evidence for EECP in Refractory Angina

ACC/AHA guidelines classify EECP as a Class IIb recommendation (Level of Evidence B) for patients with treatment-resistant angina who are not candidates for revascularization. In IEPR patient registry data, a significant portion of patients with severe angina (CCS Class III/IV) reported improvement in symptom class after treatment, and this improvement was observed to persist in some patients throughout follow-up.

EECP Does Not Produce the Same Result in Every Patient

Refractory angina is a complex clinical picture, and the effect of EECP can vary from person to person. While some patients report significant symptom reduction, others may not see the expected benefit. For this reason, EECP should be viewed not as a "definitive solution" but as an option that may be considered following individual evaluation.
